- Provide skilled nursing care such as wound care, catheter care, medication administration, and symptom management
- Conduct skilled observation, assessment, and evaluation
- Deliver patient and caregiver education and support
- Collaborate with healthcare providers for cohesive, quality care
- On-call rotations required
- Washington LPN license (current and in good standing)
- Home Health or Hospice experience preferred-but not required!
- Current BLS certification (AHA)
- Valid driver's license, insured vehicle, and reliable transportation
