Position Summary
Village Home Care is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to join our growing home health team. The LPN will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse and physician.
Key Responsibilities
* Provide direct patient care in accordance with physician orders and care plans
* Monitor and document patient condition, vital signs, and progress
* Administer medications and treatments as ordered
* Educate patients and caregivers on disease management and home care techniques
* Assist with wound care, catheter care, diabetic care, and other skilled nursing services
* Communicate changes in patient condition to supervising RN and physician
* Complete timely and accurate clinical documentation
* Participate in interdisciplinary team meetings and care coordination
Qualifications
* Current Florida Licensed Practical Nurse (LPN) license (required)
* Minimum of 1 year nursing experience (home health experience preferred)
* Strong clinical, organizational, and communication skills
* Ability to work independently in a home setting
* Reliable transportation and valid driver’s license
* Current CPR certification
