A leading healthcare organization is seeking a compassionate and dedicated Home Health Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in the home setting. Responsibilities

• Provide hands-on nursing care to patients in their homes using the nursing process and established clinical standards and protocols
• Deliver patient care as ordered, including IV infusions, infusion pump management, and tube feedings under the supervision of a Registered Nurse
• Implement and maintain patient and caregiver education plans under the direction of a Registered Nurse
• Monitor patient conditions and communicate treatment responses and changes in status to the supervising Registered Nurse
• Administer medications and evaluate patient responses when appropriate
• Collaborate with interdisciplinary team members to ensure continuity and quality of care
• Maintain accurate and timely documentation of patient assessments and care provided
• Promote patient safety, independence, and positive health outcomes in the home environment

Qualifications

• Graduate of an accredited Practical Nursing program
• Current Licensed Practical Nurse (LPN) license in the state of practice
• Current Basic Life Support (BLS) certification
• Valid Florida driver's license or ability to obtain one within 30 days of hire
• Associate Degree or Diploma in Nursing required
• Minimum of one year of Licensed Practical Nurse experience required
• Previous home health or home care experience preferred
• Strong communication, organizational, and patient education skills
• Ability to work independently and effectively manage patient visits
• Must meet all applicable background screening requirements
