RN Case Manager (Home Health & Hospice)

Location: South Franklin County and Madison County.

Full Time. Monday–Friday, 8 AM–4 PM. Flexible schedule. On-call rotation with bonus pay.

Mileage reimbursement: $0.72 per mile.

Salary: $72,603–$121,005 annually.

Position Summary

We seek a dedicated registered nurse to oversee case management for hospice patients, coordinating individualized care plans and supporting families.

Responsibilities

• Provide skilled nursing care and case management for hospice patients.
• Coordinate and develop individualized care plans.
• Collaborate with interdisciplinary teams to deliver high‑quality care.
• Maintain accurate and timely documentation.
• Respond to on‑call duties with optional bonus pay.

Qualifications

• Graduate of an accredited nursing program with a current RN license.
• Minimum two years of RN experience; hospice or home‑health experience preferred.
