What You'll Do
As a Home Health RN, you'll deliver skilled, individualized nursing care to patients in the comfort of their own homes- coordinating care, building trust , and making a measurable impact every day.
Key Responsibilities
- Delivers skilled nursing care to patients in their home in accordance with individualized care plans.
- Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
- Coordinates services, informing the physician, management, and other personnel of changes in the patient’s condition and needs.
- Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
- Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
- Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement.
- Responds promptly to emails and phone calls
What We Are Looking For
- Current RN license in the state of Massachusett
- Strong technology skills
- Reliable transportation, valid driver's license, auto insurance
