Position Summary: Home Health Aid/ CNA Job Description

n

A Home Health Aide is a non-licensed member of the home care team who assists the client with the tasks of daily living as outlined in a written care plan that is established by a the Administrator or CSS and is kept in the clients home. Home Health Aides are supervised by the Administrator agency and receive scheduling direction from the agency staffing Coordinator.

Qualifications:

  1. A Home Health Aide must have a minimum of one year of personal care experience
  2. n

which is verifiable through three work references and must complete and pass this

n

agency’s employment and competency process.

n

  1. Must be at least 18 years of age.

  3. A Home Health Aide must have training as a Home Health Aide and complete a

competency evaluation.

  1. Must be physically able to perform the duties of the position, including lifting unspecified

equipment for client or managing client environment. Transferring, turning and

Essential Functions:

n
    n
  1. The HHA shall be assigned to a client by an RN. Written instructions for client care
  2. n
n

shall be prepared and updated as needed by an RN.

n
    n
  1. The HHA may provide services that are ordered by the physician in the plan of care and
  2. n
n

that the HHA is permitted to perform under state law and that are consistent with the

n

HHAs training.

n
    n
  1. HHA services provided to 4 Your Peace of Mind Home Health clients may include, but
  2. n
n

not be limited to, the following duties:

n

a. The provision of hands on personal care including, but not limited to assisting

n

clients with personal grooming and hygiene such as skin, mouth, hair care,

n

shaving, dressing and bathing.

n

b. Performance of simple procedures as an extension of therapy or nursing services;

n

c. Assisting clients in and out of bed, assisting with ambulation or exercises.

n

d. Nutrition/hydration and assisting clients with eating.

n

e. Assisting clients with toileting and elimination.

n

f. Performing household services essential to health care at home which will

n

facilitate the clients self-care at home and are necessary to prevent or postpone

n

institutionalization.

n

g. Assisting clients with medications which are ordinarily self-administered. Refer

n

to 4 Your Peace of Mind Home Health Administration of Medication Policy for

n

requirements for the provision of medication reminding services for clients who

n

can self-direct.

n

Supervision of self-administered medication in the home is limited to the

n

following:

n

● Obtaining the medication container from the storage area for the client.

n

● Ensuring that the medication is prescribed for the client.

n

● Reminding the client that it is time to take the medication as prescribed,

n

and;

n

● Observing the client self-administering the medication.

n

a. Performing other activities delegated and taught by an RN for a specific client in

n

accordance with Connecticut Statutes and Administrative Codes and 4 Your

n

Peace of Mind Home Health Nursing Service Policies. These may include such

n

services as:

n

● Assisting with changing colostomy bags/reinforcement of dressing.

n

● Assisting with the use of devices for aid to daily living, such as a

n

wheelchair or walker.

n

● Assisting with prescribed range of motion exercises.

n

● Assisting with prescribed ice cap or collar.

n

● Doing simple urine tests for sugar, acetone, or albumin.

n

● Measuring and preparing special diets.

n

● Measuring intake and output of fluids.

n

● Measuring temperature, pulse, respiration or blood pressure.

n

b. Observing appearance and gross behavioral changes in the client and reporting

n

changes in the client;s condition and needs to the RN case manager or Home

n

Health Clinical Manager.

n

c. Completing records regarding services performed. Documentation is submitted

n

by end of business day.

n

d. The HHA may NOT provide the following services:

n

● Change sterile dressings.

n

● Irrigate body cavities (i.e. giving an enema, irrigate a colostomy or wound,

n

perform a gastric irrigation or enteral feeding).

n

● Catheterize a client.

n

● Administer medication.

n

● Apply heat by any method.

n

● Care for a tracheostomy tube.

n

● Provide any person health service which has not been included in the plan

n

of care.

n

  1. In addition to the requirements outlined in the 4 Your Peace of Mind Home Health
  2. n
n

Orientation Policy, the HHA and HM shall be oriented and demonstrate competency in

n

any service the employee or contractor is to perform prior to providing client care.

n

Competency assessments shall be documented by the Home Health Clinical Manager or

n

another RN who possesses a minimum of two (2) years of “hands on” clinical nursing

n

experience (direct provision or care and/or supervising nursing services or teaching

n

nursing skills in an organized curriculum or in-service program), at least one (1) year of

n

which must be in the provision of home health care. Subject areas must include, but not

n

be limited to the following:

n

a. Ethics, including confidentiality of information.

n

b. Client Rights and Responsibilities.

n

c. Communication skills, including the ability to read, write and verbally report

n

clinical information to clients, representatives, and caregivers, as well as to other

n

Agency staff.

n

d. The physical, emotional, and developmental needs of and ways to work with the

n

populations served by 4 Your Peace of Mind Home Health, including the need

n

for respect for the patient, his or her privacy, and his or her property.

n

e. Consumer control and the independent living philosophy.

n

f. Instrumental activities of daily living.

n

g. Observation, reporting and documentation of client status and the care or service

n

furnished.

n

h. Recognizing changes in the consumer that need to be addressed.

n

i. Handwashing and 4 Your Peace of Mind Home Health Infection Prevention and

n

Control policies and procedures.

n

j. Universal precautions.

n

k. Body mechanics.

n

l. Maintenance of a clean, healthy and safe environment.

n

m. Fire safety.

n

n. Recognizing emergencies and knowledge of instituting emergency procedures

n

and their application. This includes knowledge of 4 Your Peace of Mind Home

n

Health policies for handling of medical and non-medical emergencies.

n

o. Assignment and supervision of services.

n

p. Reporting complaints or grievances.

n

q. Recognizing and reporting abuse or neglect.

n

r. Dealing with difficult behaviors.

n

s. Each employee’s specific duties and responsibilities.

n

t. The roles of, and coordination with other community service providers.

n

Health for an HHA who will provide personal care must address the following

n

additional subject areas:

n

a. Bathing (sponge, tub, or shower bath), shaving, grooming and dressing.

n

b. Hair care (including shampoo in sink, tub or bed), skin, nail and mouth care.

n

c. Assistance with safe ambulation and transfer techniques.

n

d. Adequate nutrition, hydration, meal preparation and feeding.

n

e. Toileting and elimination.

n

f. Reading and recording temperature, pulse and respiration.

n

g. Assistance with medication reminders for medications that are normally self-

n

administered. This includes but is not limited to what constitutes medication

n

reminding and its specific limitations.

n

h. Appropriate and safe techniques in personal care tasks.

n

i. Normal range of motion and positioning.

n

j. Methods and techniques to prevent skin breakdown, contractures and falls.

n

k. Basic elements of body functioning and changes in body function that must be

n

reported to the Support Services Supervisor or Home Health Clinical Manager.

n

l. Any additional training identified by 4 Your Peace of Mind Home Health which

n

is necessary to meet the needs of the population served by the agency.
