  Assisting clients with daily living activities, including bathing, grooming, dressing and feeding.

• n

  Assisting patients with mobility and transfers.

  n

• n

  Completing necessary documentation accurately and promptly.

  n
n
• n

  Communicating effectively with clients, families and other healthcare professionals regarding client needs and concerns.

  n
n
• n

  Following established care plans and contribute to the development of personalized care strategies.

  n
n

Job Requirements

• n

  High School Diploma or GED

  n
n
• n

  CNA-Completion of 75, 120 or 150 hour Certified Nursing Assistant Pgm/Must be in State C.N.A registry

  n

• n

  HHA-Successful completion of a competency evaluating program

  n
n
• n

  CNA-One year of Certified Nursing Aide-Home Health Hospice

  n
n
• n

  HHA-One year home health aide, patient tech or other related experience and/or passes CHI Health at Home's competency

  n
n

Pay Range

$17.24 - $24.35 /hour
