Personal Home Care of North Carolina (PHC) is a rapidly growing full‑service Home Care Agency looking for energetic and dedicated caregivers that can fulfill the clinical needs of our growing patient population. We are growing quickly and looking for a dedicated Home Care Nurse to provide excellent patient care and employee supervision and training.

Position: Part‑time, as needed Mon‑Sun
Service Area: Charlotte, NC

Home Care Registered Nurse is responsible to perform general nursing duties for patients under the direction of a Physician at the patient’s home.

Home Care RN Job Functions and Schedule

• Provision of nursing care; including assessments, treatments, diagnostic procedures, and patient education based on plan of care and physician’s orders.
• Observe signs and symptoms, reactions to treatment, drug response/interactions and changes in the patient’s condition; with appropriate follow up in care and reporting to supervisor or physician as needed.
• Coordinate care, creating plans of care for in‑home aides as needed with patient/responsible party, physician, other disciplines and internal and external resources.
• Document accurately, objectively and timely in accordance with regulations and agency policy/procedures.
• Perform supervisory visits for patients and in‑home aides.
• Perform training and orientation for all newly hired in‑home aides annually and at hire.
• Participate in multi‑disciplinary team meetings weekly.
• Participate in learning and job‑enhancement activities as directed by the supervisor.

Home Care RN Job/Qualification Requirements

• Prior Home Care Experience or Educator experience is a plus (2+ years).
• A positive attitude.
• Punctuality.
• Excellent time management, organizational and reporting skills.
• Strong communication skills, both written and verbal.
• Excellent planning, organizational and reporting skills.
• Bi‑languistic (Russian/English; Spanish/English) is a plus.
• Pediatric experience is a plus.
• Current NC or multi‑State RN License.
• Current driver’s licence and automobile liability insurance issued in NC.
