Park Nicollet is looking to hire a Physical Therapist to join our Home Care/Hospice team. The role provides skilled therapy in patients' homes and acts as a case manager to assess needs and develop comprehensive care plans in collaboration with families and the care team.
Complete licensure in Minnesota, BLS certification, and a degree in Physical Therapy are required. Work four days a week, Monday through Friday, 8:00 am–4:30 pm, with occasional weekend duties.
