Home Care Assessment Nurse

BrightStar Care is seeking a compassionate and experienced Home Care Assessment Nurse to join our team. In this role, you will conduct in-home assessments for clients to determine their care needs and help develop individualized care plans that support safe, quality care at home.

Responsibilities

• Conduct comprehensive nursing assessments in clients' homes.

• Evaluate clients' medical, functional, and personal care needs.

• Develop and update individualized care plans based on assessment findings.

• Communicate with clients, families, physicians, caregivers, and the BrightStar Care team.

• Document assessments and clinical information accurately and timely.

• Identify changes in a client's condition and communicate appropriate recommendations.

• Provide education and guidance to clients and their families.

• Follow all applicable clinical, safety, and regulatory standards.

Qualifications

• Current, unrestricted RN license .

• Previous home care, home health, or clinical assessment experience preferred.

• Strong clinical assessment and documentation skills.

• Excellent communication and organizational skills.

• Reliable transportation and ability to travel to clients' homes.

• Compassionate, dependable, and professional approach to client care.
