Northwestern Medicine is seeking a Nurse Practitioner or Physician Assistant to support Primary Care at Home. The APP will travel to patients' homes in the western suburbs of Chicago, or see patients in assisted living facilities.
Schedule is Monday–Friday with some after-hours on-call. Responsibilities include comprehensive patient assessments, direct clinical care, prescribing, and coordination with collaborating physicians.
