Gentiva Hospice seeks a compassionate Hospice RN / RN Case Manager to lead in-home nursing care and guidance for patients with life-limiting illness. You will work with families to implement individualized Plans of Care across home and other settings, delivering skilled nursing interventions and education.
You will collaborate with physicians, LPNs, CNAs, social workers, chaplains, and the broader IDG to ensure coordinated, timely, and evidence-based care while maintaining accurate documentation
