Behavioral Health Case Manager

Position Details
Contract Length : 13 Weeks
Schedule : 32 Hours/Week
Shift Timing : 10:00 AM - 06:30 PM
Days : Wednesday - Saturday
W2 Pay Rate : $30/hr
Key Responsibilities

• Manage assigned caseload for individuals recovering from substance use disorders
• Conduct intake assessments, treatment planning, and aftercare coordination
• Support medication management and safety planning needs
• Coordinate with families, referral sources, and community providers
• Assist with transportation and continuing care arrangements
• Complete documentation and case management records in a timely manner
• Support crisis intervention, de-escalation, and milieu management
• Facilitate educational, wellness, and recovery-focused group activities
• Help identify aftercare resources and community support services

Requirements

• Bachelor's Degree or Associate Degree in Human Services or related field
• 2-3 years of experience in chemical dependency, behavioral health, or substance use treatment preferred
• Knowledge of community recovery and aftercare resources
• Certified Addiction Counselor (CAC) preferred
• Ability to complete CPI and CPR certification/training
• Strong communication, documentation, and crisis management skills
