Opportunity: PACE Home Care Coordinator HHA
*Full Time Direct Hire
*Work under the supervision of PACE Care Coordination Manager to provide foundational support to the Home Care Coordination team.
*Duties: Conduct home safety evaluations, coordinate and monitor IDT approved services for PACE participants. Coordinate IDT approved meal delivery to client homes, coordinate IDT approved Durable Medical Equipment and incontinence supply delivery to client homes and coordinate appropriate caregiver services, complete Home Care Coordination care plan at enrollment and semi-annually thereafter. Conduct pre-enrollment, annual, semi-annual and change of address home safety evaluations. Document and track actions, services and delivery status in EMR system, in accordance with clinical DHCS, CMS regulations and guidelines. Communicate regularly with participants, their families, and other healthcare providers to ensure coordinated care delivery.
*Requirements: Active CNA certification and CA Public Health Home Health Aid (HHA), CPR and First Aide certification from American Heart Association, 1 year of experience with the frail or elderly population, Valid CA Drivers License.
