- Appointment Times:
- New Patients: 30 minutes
- Follow-ups: 15 minutes
- Board Certified in Hematology or Medical Oncology
- Hematology/Oncology Fellowship Required
- KY License or IMLC (LOQ in hand preferred)
- BLS & ACLS Required
- Experience with outpatient/inpatient Hematology & Oncology
- Intrathecal Chemotherapy Experience Required
- Intracavitary Chemotherapy Experience Preferred
- NPDB Self-Query Report (within 30 days) Required
- COVID Vaccination Required
