Healthy Planet Analyst

HDM is seeking an experienced Epic Healthy Planet / Compass Rose Analyst to support a healthcare organization with population health, quality, and value-based care initiatives. This role will provide hands-on functional support for Epic Healthy Planet and Compass Rose, including Care Gaps, Health Maintenance, HEDIS and quality measures, population health workflows, and related reporting and analytics.

. The analyst will assist with workflow validation, testing, issue identification and resolution, and go-live support related to the Healthy Planet/Compass Rose environment.

Responsibilities

• Provide functional and operational support for Epic Healthy Planet and Compass Rose within a population health environment.
• Support HEDIS, quality measures, and value-based care analytics, including the identification and management of care opportunities.
• Support Care Gaps and Health Maintenance workflows and assist with maintaining appropriate population health functionality.
• Analyze and troubleshoot Healthy Planet workflows, configuration, data, and reporting issues.
• Work with clinical and operational stakeholders to understand population health requirements and translate them into Epic workflows and solutions.
• Support testing activities, including test script execution, workflow validation, issue documentation, defect resolution, and regression testing.
• Assist with the PaSSPort implementation and go-live, supporting the consolidation of three Epic instances into one.
• Validate that Healthy Planet/Compass Rose workflows, measures, care gaps, and related functionality are operating appropriately following the instance consolidation.
• Partner with application, clinical, reporting, and project teams to investigate issues and coordinate resolutions.
• Document workflows, findings, configuration considerations, and outstanding issues as needed.
• Participate in project meetings and provide status updates related to assigned Healthy Planet/Compass Rose workstreams.

Qualifications

• Hands-on experience supporting Epic Healthy Planet in an analyst or application support capacity.
• Strong understanding of population health workflows within Epic.
• Experience with Epic Compass Rose strongly preferred.
• Experience supporting HEDIS, quality measures, value-based care, care gaps, and/or health maintenance functionality.
• Experience with Epic testing, implementations, upgrades, conversions, or go-live support.
• Understanding of how population health data, measures, care gaps, and clinical workflows interact within Epic.
• Experience troubleshooting application and workflow issues and working with technical and clinical teams to resolve them.
• Strong analytical and problem-solving skills with the ability to investigate issues and identify workflow impacts.
• Ability to work independently in a remote environment while collaborating effectively with clinical, operational, technical, and project stakeholders.
• Strong written and verbal communication skills with the ability to document technical and workflow information clearly.
