Supporting the Senior Care team, the full-time remote Health Navigator will assist members in transitioning home by coordinating follow-up appointments, reviewing medication changes, and providing resources to manage their health effectively. Key responsibilities Conduct educational calls to inform members about available benefits and assess health needs Reach out to members with care gaps, encouraging compliance and assisting with provider location and appointment scheduling Manage system work queues and screen members for eligibility and prior case activities Required qualifications Associates Degree in education, communication, health-related field, or equivalent work experience At least 2 years of experience in a customer service support role Knowledge of medical terminology and healthcare processes Ability to work independently with minimal supervision Strong organizational and time management skills
