Job Title: Senior LTSS Service Care Manager (RN)

Duration: 06 months Contract (Potential to Extend)

Desired Start Date/End Date: 4/14/2025 - 10/10/2025

Location: Remote - Source from Laredo, TX

Shift Type: M-F 08:00am - 5:00PM (with possibility of OT when warranted)

Job Duties:

1. This position is for the Laredo area to complete assessments in member’s homes as well as completing documentation as per State and Company guidelines to ensure compliance.
2. M-F visits 3 days out of the week, with OT possible with manager approval, documentation days 2 days out of week, traveling to members’ homes to complete assessments/documentation.
3. Face to Face assessments completed 3 days out of the week with 2 documentation days, completing tasks, submitting all forms/assessments within turnaround times, completing outreaches.
