Engaging with members to establish effective relationships, the full-time hourly Member Health Assessor will conduct health risk assessments, coordinate care plan logistics, and provide health education while working remotely. Key responsibilities Conduct health risk assessments by gathering information from members, families, and providers Coordinate logistics to support care plan goals and address immediate member concerns Contribute to the development and implementation of care plans based on assessment outcomes Required qualifications High School Diploma or General Education Diploma (GED) is required Minimum of two years of experience working with at-risk populations in community settings is preferred Proficiency in Microsoft Office, including Outlook, Word, and Excel Experience working within diverse cultures and with sensitivity to their needs Ability to maintain confidentiality and demonstrate ethical conduct in all interactions
