Providing care coordination and support for at-risk patients, the full-time Health Advisor will manage inbound and outbound telephonic outreach to promote patient engagement and improve health outcomes while working a hybrid schedule of 3 days remote and 2 days in-office. Key responsibilities Assist physician practices in identifying and achieving quality care goals through direct patient outreach Collaborate with integrated care teams to support care coordination and ensure timely disease-specific and wellness visits Motivate patients to take proactive steps in their healthcare and facilitate access to appropriate care resources Required qualifications Prior experience with Electronic Medical Record systems (e.g., Epic, Athena) and willingness to learn internal tools Background in patient outreach across diverse demographics, including transition of care and patient advocacy Ability to work a hybrid schedule (3 days remote, 2 days in-office) Comfort with navigating new platforms and using in-house digital tools Capability to evaluate complex patient cases and determine appropriate resources for escalation
