General Summary: Under the Supervision of the Center Supervisor, the Family Service Worker recruits and enrolls families, connects families to resources, and provides ongoing support to improve child and family well- being, focusing on building relationships and promoting family engagement.
Principal Duties and Responsibilities: 1. Develops and implements individualized family support plans, assessing the family' s health, education, and social service needs.
2. Assist families in completing original enrollment applications for their child or children and ensure all necessary documents for enrollment are collected and filed with the application.
3. Develop, implement, follow up, and document the family' s Individualized Family Partnership Agreement, including Family goal setting
4. Accessing and documenting community services and resources: Emergency or crisis assistance in food, housing, clothing, and
transportation areas.
Education and other appropriate interventions, including opportunities for parents to participate in counseling programs or to receive information on mental health issues that put families at risk.
Provide families with opportunities for continuing education, employment training and other employment services through formal and informal networks in the community.
Services to pregnant women enrolled in programs serve pregnant women, infants, and toddlers.
Parent involvement, including child development and education. 5. Work collaboratively with all families to identify and continually assess,
either directly or through referrals, services and resources that are responsive to each family' s interests and goals, including services for children with disabilities.
6. Schedule and/ or document appointments for medical, dental, and
disability screenings, evaluations, and complete follow- ups as needed. 7. Monitor classroom absences to ensure an average daily attendance rate of

8. Secure resources within the community; complete In- Kind forms for all donated goods, services, and volunteer hours.
Attend and assist with parent committee meetings within their assigned caseload; encourage family participation.
9. Conduct( 2) home visits per school year or more as needed, and follow- up visits will be conducted in person to complete the Family Partnership Agreement.
10. Provide ongoing support and guidance to families, including home visits and regular communication.
11. Makes periodic home visits ( Health, Social Service), in October and April, as needed, to keep parents informed of the child' s health screening results and appointments, and to stay abreast of the current needs and concerns of the family. Responsible for all documentation of such visits.
12. Connect families to community resources and services. 13. Assist families in setting goals and developing action plans to achieve
those goals. 14. Will facilitate monthly parent training and arrange qualified presenters in
the areas of interest expressed by the parents. 15. Utilizes the Family Needs Assessment/ Family Partnership Agreement to
determine what the families' immediate needs are and where their areas of
Interest is concentrated.
16. Provide advanced written notification to parents regarding screening or follow- up services and development forms as soon as the appointment has been scheduled, and provide results immediately following the service.
17. Ensure parent training information is included on center parent calendars.
18. Assesses Nutrition Data record and graphs height and weight on Child Plus tracking and makes referrals, if needed.
19. Record according to Performance Standards and Child Health Record Guidelines, hemoglobin/ hematocrit obtained for each child' s health record, and
make referrals, if necessary. 20. Review child immunization records and implement procedures to bring
the child up- to- date on their status. Must obtain written documentation of parents' refusal of immunization.
21. Maintains accurate records of family interactions, services provided, and progress toward goals.
22. Maintain all children' s files current with all documentation related to the
child.
23. Maintain accurate records in the Child Plus system to ensure all Health/ Social Services are completed.
24. Work collaboratively with teachers, other Head Start preschool staff, and community partners to ensure comprehensive services for families.
25. Maintains strict confidentiality regarding children, families, and other staff inembers. Must sign a statement of Compliance or Confidentiality.

by the Parent Family Community Engagement Specialist and Center Supervisor. 27. Adheres to guidelines set in the Muscogee ( Creek) Nation, Head Start
Policies and Procedures, Oklahoma Department of Human Services: Child Care
Licensing Requirements, Federal, and State Regulations as related to Head Start. 28. Must report suspected child abuse, neglect, etc., as
mandated by the Muscogee ( Creek) Nation Policies, Tribal State and Federal laws.
29. Must sign a statement of understanding and verification of training on reporting procedures and the responsibility of reporting abuse, neglect, etc. 30. Before leaving the work site for the Head Start Preschool business. Staff
must sign in and out on the activity leave form. 31. Must be willing to stay after hours, if necessary, to Complete an important task or meeting. 32. Must attend in- state and out- of-state workshops, conferences, and
trainings upon request by the Manager. 33. Must attend all mandatory trainings, workshops, and professional
development
34. Implement the knowledge and techniques gained through training and workshops.
35. Must be familiar with the Head Start Performance
Standards, the Head Start Act, and be willing to participate in on- site reviews.
36. Must have a physical annually and an initial TB skin test 37. Must obtain a Commercial Driver License ( CDL) within the first six ( 6)
months of employment.
38. Must obtain within 18 months of hire a credential or certification in
Social Work, Human Services, Family Services, Counseling, or a related field. 39. Performs all other related duties as assigned by the Center Supervisor,
Program Coordinator/ Specialist, and/ or Manager related to program philosophy.
Minimum Requirements: Education - High School or GED
Experience - Working with Families and Community.
Preferred Requirements: Education - Credential or Certification in Social Work, Human Services, Family Services, Counseling or a related field.
Preference: Muscogee and Indian preference.
