About The Role

The Head of Care Coordination role at Chronius is pivotal to our unique care delivery model. Our approach melds empathy, deep condition expertise, and exceptional care coordination. We offer a care experience for our patients where managing healthcare no longer feels like a full-time job, thanks to our proactive care coordination team that lifts the heavy burden from our patients' shoulders.

Care coordination is crucial for our patient population. It simplifies navigating the fragmented healthcare system, ensures all care providers are synchronized, and facilitates effective communication and data sharing. This approach prevents duplicative tests and services, aligns care with patient preferences, increases patient compliance with their care plans, and enhances safety and outcomes. Using advanced IT health tools and team-based care, we ensure seamless care transitions and emphasize follow-up care to manage chronic conditions effectively. Beyond clinical coordination, we address social determinants of health and empower patients as active participants in their care, making Chronius a true partner in our patients' healthcare journeys.

Key Responsibilities:

  • Manage a team that executes a wide range of care coordination tasks, processes inbound patient communications, and ensures seamless continuity of care across all stages. Ensure timely and effective exchanges between patients and their care team, enhancing patient engagement and satisfaction.
  • Oversee pre-appointment preparations and post-visit care coordination, streamlining processes to prepare providers and patients for appointments. This includes ensuring providers have the necessary patient information and history in hand and ensuring patients have completed intake forms.
  • Support patients by providing education on conditions and treatments, managing chronic conditions, and addressing social determinants of health.
  • Step in to do hands-on care coordination work such as patient messaging triage as needed to supplement the efforts of the care coordination team.

Operational Efficiency:

  • Collaborate with the tech team to implement automations and digital tools to streamline care coordination workflows, enhancing overall efficiency.
  • Continually refine operational workflows and policies by incorporating efficient practices that aim for excellence in patient care and team productivity.

Team Communication and Collaboration:

  • Lead effective communication strategies and data sharing between the clinical and tech teams, ensuring seamless collaboration and up-to-date knowledge on patient care.
  • Coordinate with external partners in mental health, nutrition, social services, and disability agencies, facilitating a holistic approach to patient care.
  • Streamline the triaging of clinical issues and the dissemination of critical updates across teams, prioritizing patient needs and ensuring efficient response to enhance care delivery and patient satisfaction.

Leadership and Team Operations:

  • Optimize team performance through clear task management, ensuring each member is fully engaged and efficiently contributing to patient care coordination.
  • Continuously improve the overarching care coordination strategy and protocols in close partnership with practice leadership, ensuring the approach is aligned with the practice’s goals and effectively addresses patient needs.
  • Work with CEO and VP of Growth and Operations to balance growth targets against hiring needs. Own recruiting and training additional care coordinators.

Provide Same-Day "Urgent Care" Availability:

  • Serve patients in same- and next-day urgent care-style visits for relatively simple emergent needs. This is highly desirable, but not required, and amounts to a very small percent of the week.

Qualifications:

The Head of Care Coordination must be able to work EST hours.

  • Educational and Professional Background: We seek a candidate with an NP or a PA degree, but are also open to candidates with advanced degrees in Healthcare Administration, Case Management, or related fields, provided they bring extensive clinical experience. An RN candidate with significant care coordination and management experience could be a fit as well. We prefer candidates who have significant experience in clinic management, patient advocacy, or care coordination, recognizing the diverse paths to expertise in care coordination.
  • Clinical Experience: A minimum of 4 years in a clinical or medical office setting is desired, demonstrating a robust ability to navigate and positively impact primary care or chronic disease management.
  • Leadership and Team Development: The ideal candidate will have demonstrated success in staffing, leading, motivating, and managing a diverse team of healthcare professionals and support staff. This includes proven ability to inspire teams towards excellence, foster a collaborative work environment, and drive initiatives that improve patient care and team efficiency.
  • Vision and Innovation: A passion for redefining healthcare delivery through innovative solutions to complex healthcare challenges is crucial. Candidates should possess a visionary mindset, with a track record of thinking outside the box and implementing transformative healthcare practices.
  • Strategic Planning and Organizational Skills: Strong organizational and strategic planning skills are essential, with an ability to oversee multiple projects, prioritize tasks, and meet deadlines.
  • Communication and Patient-Centered Care: Exceptional communication skills are required, both written and verbal, to effectively coordinate care between patients, healthcare providers, and external partners. A deep understanding of patient-centered care principles, including empathy, patient advocacy, and holistic care, is vital.
  • Technical Proficiency: Proficiency in using Electronic Health Records (EHRs) and digital communication tools is necessary. Experience in optimizing workflows technology-related workflows for care coordination purposes is highly desirable.
