Role Description
This is a full-time on-site role based in Great Neck, NY, with travel to our practice sites across Nassau, Suffolk and Queens Counties. Together we will determine the exact title, but we are looking for someone to serve as the Head, Care Management & Clinical Programs/Innovation (Medicare patients), reporting to the CEO/COO. The role involves leading care management and clinical innovation initiatives focused on Medicare patients. Key responsibilities include developing & managing clinical programs, from inception to beta testing and scaled rollouts. The position also ensures compliance with regulatory guidelines and the delivery of patient-centric solutions for optimal health outcomes such as Medicare's Chronic Care Management, Remote Patient Monitoring, and Advanced Primary Care Management.
Qualifications
- Bachelor’s degree in a relevant clinical field is required (RN, NP or PA highly preferred, but other clinical disciplines encouraged to apply) Non-Clinicians with MHA/MBA/MPH will be considered.
- Proven skills in Program Development & Management to write proposals, program documents and related policies and procedures to optimize care management initiatives in-between office visits, during hospitalizations, after ER visits and more.
- Strong Primary Care Physician Stakeholder Management abilities to coordinate effectively with physician partners and build an integrated care team
- Shared belief & vision that the healthcare system for seniors is overly complex and broken, and the deep-rooted, entrepreneurial passion to fix it
- Demonstrated Team Leadership experience to mentor and guide cross-functional teams towards organizational goals.
- Strong technical writing skills to create proposals, new programs, policies and educational materials
- Strong oral communication skills to engage physician stakeholders along with active listening to hear their ideas, wants and needs to better manage their Medicare patients
- Direct clinical skills or substantive experience working with chronic disease management programs to oversee and help guide the clinical team
- Exceptional knowledge of Medicare care models aimed to improve the quality of care, optimize Medicare Beneficiaries quality of life, all while reducing the cost of care
Compensation
The compensation will be based on the individual's credentials, experience and expertise, and is anticipated to be in the range of $142,000-$175,000 plus an annual performance bonus + full benefits after a 90-day waiting period.
