nAs the Harm Reduction Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:

Conduct outreach activities to engage the target population

Identify community partners to engage PWUD who are unaware of their status or untreated
n n - n
Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks
n n - n
Provide enrollment into the agencies Syringe Exchange Program
n n - n
Facilitate brief assessments with the target populations
n n - n
Through review of the assessment, identify community based services to provide client linkage
n n - n
Facilitate linkage to services through escort as necessary
n n
Harm Reduction Case Manager Minimum Salary: $42,000
nHarm Reduction Case Manager Maximum Salary: $42,000
nHarm Reduction Case Manager Schedule: Mon – Fri 8:30am – 4:30pm
nQualifications:

High School Diploma or GED

1-3 years of relevant experience, Human Services

Must have valid Driver's License and access to reliable vehicle
