About the Role


Anuco Home Health Care LLC is seeking compassionate, dependable, and highly skilled Physical Therapists and Physical Therapist Assistants to provide patient-centered rehabilitation services in patients’ homes. This role is ideal for professionals who value clinical independence, flexible scheduling, strong communication, timely documentation, and the opportunity to help patients improve their mobility, safety, and independence.


Candidates must be comfortable traveling throughout their selected territory, managing their schedules, communicating changes in patient condition, and completing accurate clinical documentation in Axxess within agency-required timeframes.


Physical Therapist Responsibilities



  • Complete comprehensive physical therapy evaluations and establish individualized treatment plans.

  • Perform ordered skilled physical therapy services in accordance with the plan of care.

  • Complete evaluations, reassessments, recertifications, OASIS assessments when applicable, and discharge summaries.

  • Evaluate mobility, strength, balance, endurance, transfers, gait, fall risk, and functional limitations.

  • Communicate patient progress and recommended treatment changes to physicians and the interdisciplinary team.

  • Provide patient and caregiver education regarding home exercise programs, assistive devices, fall prevention, and safe mobility.

  • Supervise Physical Therapist Assistants and Home Health Aides as assigned and required.

  • Enter complete, accurate, and visit-specific documentation in Axxess within agency-required timeframes.

  • Respond promptly to documentation corrections and quality-assurance requests.


Physical Therapist Assistant Responsibilities



  • Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.

  • Follow the established plan of care and immediately report concerns or recommended changes to the supervising PT.

  • Assist patients with therapeutic exercises, gait training, transfers, balance, strengthening, endurance, and functional mobility.

  • Educate patients and caregivers regarding home exercise programs, assistive devices, and safety precautions.

  • Document every visit accurately in Axxess, including treatment provided, patient response, progress toward goals, education, and care coordination.

  • Complete and submit visit documentation within agency-required timeframes.

  • Promptly report changes in condition, falls, hospitalizations, incidents, complaints, or unmet needs.

  • Participate in care coordination, case conferences, QAPI activities, and required agency training.


Requirements



  • Current, unrestricted Illinois PT or PTA license appropriate to the position.

  • Graduate of an accredited Physical Therapy or Physical Therapist Assistant program.

  • Home health experience strongly preferred.

  • Axxess Home Health EMR experience preferred.

  • Current CPR or BLS certification.

  • Valid driver’s license, reliable transportation, and current automobile insurance.

  • Professional liability insurance as required.

  • Strong clinical judgment, communication, organization, and time-management skills.

  • Ability to work independently and travel to patient homes.

  • Ability to complete documentation accurately and within agency-required deadlines.

  • Ability to respond promptly to QA corrections, missing-documentation requests, and clinical follow-up.

  • Commitment to HIPAA, patient safety, professional boundaries, and agency policies.


Clinical Territories


Applicants may select one or more territories they can cover consistently.


East/Central Territory: Chicago, Oak Park, River Forest, Elmwood Park, and Cicero.


Territory assignments are based on patient location, clinician availability, payer requirements, clinical competency, and agency needs.


Documentation Expectations



  • Document each visit completely and accurately in Axxess.

  • Ensure documentation reflects the skilled service provided, patient response, progress toward goals, and continued need for care.

  • Submit evaluations, reassessments, routine visit notes, recertifications, and discharge documentation within established deadlines.

  • Complete documentation corrections promptly after receiving QA feedback.

  • Notify the office immediately if a technical, scheduling, or clinical issue may delay documentation.

  • Maintain documentation that supports physician orders, visit frequency, payer requirements, and the established plan of care.

  • Incomplete or late documentation may delay care coordination, authorization, billing, and future patient assignments.


Performance Expectations



  • Accept only assignments they can service consistently and safely.

  • Contact the office immediately when scheduling or patient-access problems occur.

  • Complete all assigned visits and documentation within required timeframes.

  • Maintain active licenses, certifications, and insurance.

  • Respond promptly to agency communications and correction requests.

  • Communicate changes in patient condition without delay.

  • Participate professionally in care coordination and supervisory communication.

  • Provide respectful, dependable, and compassionate care in every patient’s home.
