Responsibilities
- Provide comprehensive outpatient primary and geriatric care
- Evaluate and manage acute and chronic medical conditions
- Develop individualized treatment and care plans
- Provide preventive care and promote healthy aging
- Coordinate care with specialists, caregivers, and community resources
- Support population health and value-based care initiatives
- Collaborate with physicians, advanced practice clinicians, nurses, medical assistants, behavioral health professionals, and other care team members
- Develop and improve community-based geriatric care programs

Shift & Schedule
- Full-Time
- Participate in shared telephone call coverage as needed
Requirements
- MD or DO degree from an accredited medical school
- Board certification or eligibility in Geriatrics, Internal Medicine, or Family Medicine
- Active and unrestricted medical license
- Experience providing care to older adults
- Strong clinical, communication, and interpersonal skills
- Commitment to patient-centered, evidence-based care
- Ability to work effectively in a multidisciplinary team
- Strong leadership and program development skills
- Interest in prevention and healthy aging
- Ability to build strong relationships with patients and families
- Spanish fluency preferred but not required
