Geriatrician or Internal Medicine Physician

CHRISTUS St. Vincent is seeking a compassionate and dedicated Geriatrician or Internal Medicine Physician to join their growing outpatient practice at the Center for Healthy Aging in Santa Fe, New Mexico. This exceptional opportunity focuses on providing comprehensive, patient-centered care for an older adult population, prioritizing quality, continuity, and relationship-based care. The role emphasizes maintaining independence, dignity, and quality of life for patients, guided by the nationally recognized 4Ms Framework.

Practice Info

• 100% outpatient practice
• Manageable patient volume of approximately 16+ patients per day
• Limited call responsibilities: Primary Care call rotation, telephone call only with nurse triage support
• Focus on providing comprehensive care for older adults in a supportive, team-based setting
• Epic health record
• Robust Nurse Navigation support

Responsibilities

• Provide comprehensive outpatient medical care for adult and geriatric patients
• Conduct preventive health screenings and wellness evaluations
• Diagnose and manage acute and chronic medical conditions
• Treat common geriatric conditions such as dementia and cognitive disorders, osteoarthritis and musculoskeletal conditions, urinary incontinence, and multiple chronic disease management
• Develop and oversee individualized treatment plans
• Coordinate care with specialists, caregivers, and interdisciplinary team members
• Order and interpret diagnostic tests and procedures
• Support patients in achieving their health goals while preserving function and quality of life

Shift & Schedule

• Flexible schedule: Monday–Friday, 8-hour days, or Four 10-hour workdays per week

Requirements

• MD or DO
• Board Certified or Board Eligible in Geriatric Medicine, or Internal Medicine
• Eligible for or currently holding a New Mexico medical license
