Sanford Health in Bismarck, ND, is seeking a BC/BE Gerontologist or Family Practice provider to join their team. This role involves serving as a primary care provider for skilled nursing facilities and assisted living facilities, working alongside a dedicated team of Physicians and APPs. Practice Info
- Work with a team of Physicians and APPs to cover nursing homes and assisted living facilities
- Serves as primary care provider for skilled nursing facilities
- Cover 8 nursing homes and 6 assisted living facilities
- Sanford nursing home outreach team consists of an additional MD partner as well as APPs to help support
- Supported by hospital system integration for sub specialties
Responsibilities
- Perform history and physical examination
- Admit (if applicable to the setting)
- Evaluate, diagnose, treat and provide consultation to patients of all ages with a wide variety of illness, diseases, injuries, mental health conditions and functional disorders of circulatory, respiratory, endocrine, metabolic, musculoskeletal, hematopoietic, gastroenteric, integumentary, nervous, female reproductive, and genitourinary systems
- Provide pre-operative care of surgical patients
- Provide post-operative medical care of surgical patients
- Address return to work issues

Requirements
- BC/BE Gerontologist or Family Practice provider
- A minimum of one year of experience is preferred
- M.D. or D.O. degree
- Successful completion of a Family Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or the Royal College of Physician and Surgeons of Canada
- Certification in the primary area of practice by the appropriate specialty/subspecialty board of the American Board of Medical Specialties (ABMS), the American Osteopathic Association (AOA), as applicable
- If not board certified at the time of application but completed residency within the last five years, must achieve board certification within five years from the date of completion of residency training
