Geriatrician

Sutter Medical Group (SMG) is seeking Primary Care Physicians to join Sutter Senior Care, a new senior-focused primary care model serving older adults with complex and changing healthcare needs throughout the Greater Sacramento area. Sutter Senior Care is designed around a different approach to primary care for older adults, emphasizing panel ownership, longitudinal relationships, proactive outreach, care coordination, and meaningful touchpoints between the care team, patients, and caregivers. Physicians will join during an important stage of growth, helping to shape a care model designed to help patients maintain quality of life while reducing avoidable emergency department visits and hospital utilization.

Practice Info

• Approximately 10 pre-scheduled appointment slots per day, using a mix of 30- and 60-minute visits
• 60-minute new-patient and complex visits
• 30-minute routine follow-up visits
• Monday-Friday outpatient practice
• Interdisciplinary, team-based approach to complex senior care
• Shared 1:3 phone call, which is expected to include evenings, weekends, and holidays

Responsibilities

• Manage a patient panel collaboratively and use different types of clinical and care-team interactions to respond proactively as patient needs change
• Collaborate with an interdisciplinary care team including APCs, nurses, licensed clinical social workers, care coordinators, community health workers, clinical pharmacists, dietitians, and physical and occupational therapists

Compensation

• $334,077 - $371,197 per year

Shift & Schedule

• Monday-Friday outpatient practice
• Customizable schedule and appointment slots
• Shared 1:3 phone call, which is expected to include evenings, weekends, and holidays
• Dedicated administrative time per week

Qualifications

• MD or DO degree from an accredited institution
• Board certified or board eligible in Internal Medicine, Family Medicine, Geriatric Medicine, or another applicable primary care specialty
• Eligible for an unrestricted California medical license
• Strong clinical diagnostic, communication, and patient-engagement skills
• Interest in caring for older adults with complex or changing healthcare needs
• Ability to practice effectively within an interdisciplinary, team-based environment
• Understanding of population health and preventive care principles
• Experience in primary care, geriatrics, or value-based care preferred
• Geriatrics fellowship training or equivalent experience strongly preferred, but not required
