Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist to join our facility. The PT will assess patients, develop and administer treatment plans, and educate patients and families. The role includes supervising Physical Therapy Assistants and coordinating with the care team in a long-term care setting.
Requirements include graduation from an accredited PT program, licensure, and documentation proficiency. Prior geriatric care experience is preferred.
