Geriatric Primary Care Launch Director

Hospice of Michigan Kalamazoo is seeking a Practice Director to lead the NorthStar Care Community EverCare geriatric primary care initiative. This role owns the provider practice build, translating clinical vision into scalable operations, staffing models, and systems that deliver high-quality, value-based care.

The Director partners with executive leadership to ensure readiness, launches the service line, and positions the practice for sustainable growth and integration across the organization.
