UofL Health is seeking a dedicated APRN to provide comprehensive primary and post-acute geriatric care in long-term care and assisted living facilities. You will manage a caseload of older adults with complex multi-system conditions and support recovery through coordinated care plans.
The role requires MSN or DNP, Kentucky RN/APRN licenses, AGPCNP/AGACNP board certification (FNP considered with relevant experience), DEA and CSR, and BLS.
