Geriatric Physician

SSM Health Monroe Hospital is seeking a Board-Certified Physician to join their established geriatric medicine team serving skilled nursing facilities in Monroe, WI. Practice Info

• Serving skilled nursing facilities across the Monroe area
• Serving southwestern Wisconsin and northern Illinois communities
• Established census - begin seeing patients immediately
• Established patient panel in long-term care and post-acute settings
• Geriatric patients with complex medical needs

Responsibilities

• Provide comprehensive medical care to residents in skilled nursing facilities across the Monroe area
• Conduct routine assessments, medication management, and care plan development
• Respond to acute changes in patient condition during your scheduled hours
• Supervise and collaborate with a team of experienced Geriatric Nurse Practitioners
• Complete documentation in the EPIC EMR system
• Participate in interdisciplinary care team meetings
• Communicate with patients, families, and facility staff regarding treatment plans

Compensation

• Salary Range: $135,000 to $857,000 / Year
• This position may also be eligible for incentive pay

Requirements

• Completion of an ACGME accredited Residency Program
• Board Certification in Family Medicine OR Internal Medicine
• Dual Licensing: WI and IL
• Active medical license in Illinois (or ability to obtain prior to start date)
• DEA certification
• Current BLS certification
• Strongly Preferred: Previous experience in post-acute care settings
• Strongly Preferred: Previous experience in long-term care or skilled nursing facilities
• Strongly Preferred: Geriatric medicine experience or specialized training
