Geriatric Physical Therapist — Growth & Impact

The Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist (PT) to join our rehabilitation team. The PT will assess patients, establish treatment programs, and supervise Physical Therapy Assistants, focusing on long-term and geriatric care while ensuring documentation accuracy and confidentiality.

Responsibilities include direct patient care, goal setting, and collaboration with physicians and other disciplines to deliver high-quality, patient-centered therapy services.
