BoldAge PACE seeks a dedicated Physical Therapist to provide comprehensive rehabilitation services to older adults in our coordinated care program. The PT will assess functional status, develop individualized treatment plans, deliver therapeutic exercises and gait training, and support fall prevention to maximize independence and safety. You’ll document in EHRs, collaborate closely with an interdisciplinary team, educate patients and families, and contribute to care planning across home and center-based settings.
