Personal Health Partners is seeking a Nurse Practitioner (Adult/Gerontology) to provide high-quality primary care in a high-touch, resident-focused model at Clay Healthcare Center in Ivydale and Braxton Healthcare Center in Sutton, WV.
The NP will diagnose, treat, and manage acute and chronic conditions, collaborate with the care team, and ensure accurate documentation in our EMR. This full-time role offers a Monday–Friday schedule with a $15k sign-on bonus and quarterly productivity bonuses.
