Community Health Network is seeking an Elder Life Specialist (ELS) to join the Hospital Elder Life Program (HELP) within the Geriatric Evaluation Management (GEM) team. The ELS will enroll high-risk inpatients and coordinate volunteers to provide Daily Visitor, Therapeutic Activities, Early Mobilization, Nutrition/Hydration, and Sleep programs.
The role requires a health science degree and 2+ years in healthcare, preferably with geriatrics.
