To support a growing healthcare team, the full-time Georgia RN Case Manager will coordinate and facilitate quality care for ESRD and CKD patients, working remotely from Monday to Friday. Key responsibilities Manage care coordination between patients, families, and healthcare teams to ensure effective treatment and minimize care fragmentation Develop and implement individualized care plans, assessing patient needs and supporting transitions across healthcare services Collaborate with interdisciplinary teams and regional operations to enhance the quality and efficiency of patient care Required qualifications A.D.N degree from an accredited nursing program required; B.S.N preferred Current Registered Nurse (RN) license in the state of practice required Minimum of five years' experience in clinical nursing, with three years in renal nursing preferred Case management experience required; alternative experience in assessing patient needs and managing care plans is acceptable Current CPR certification and a valid driver's license with a positive driving record required
