To provide high-quality, preventive-focused care through telehealth, a Georgia Licensed Telehealth Physician will deliver routine visits to Medicare patients, manage personalized care plans, and collaborate with an integrated care team in a fully remote, full-time position with no on-call or weekend duties. Key responsibilities Provide routine telehealth visits with Medicare patients, focusing on E/M and Chronic Care Management Create and manage personalized care plans for ongoing conditions Collaborate with the integrated care team to ensure thorough documentation and compliance with quality measures Required qualifications MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics Active medical license; multi-state licensure is a plus Credentialed with major insurance plans such as UHC, Aetna, and Humana Strong interest in preventive care, senior health, or chronic condition management Tech-savvy and comfortable practicing via telehealth
