Seeking a compassionate and clinically skilled Georgia Licensed Nurse Practitioner, the full-time remote Telehealth Provider will assess Medicare beneficiaries experiencing urgent clinical concerns, provide treatment recommendations, and collaborate with nursing and healthcare teams to ensure effective care management. Key responsibilities Conduct telehealth evaluations for Medicare beneficiaries with urgent, non-emergent clinical concerns Document encounters accurately in the electronic medical record and provide patient education on disease management Collaborate with nursing staff and other healthcare providers to manage care and reduce avoidable emergency department visits Required qualifications Active, unrestricted Nurse Practitioner license, Georgia preferred National certification appropriate to specialty Minimum of 3 years of clinical practice experience in relevant fields Experience managing hypertension and common chronic conditions Prior telehealth or virtual care experience
