Right At Home Care Team MemberThe right individual will share our vision for providing exceptional care for seniors and those with disabilities.The RN is charged with determining the appropriateness of potential clients and home supervisory visits of current clients.Job duties include:Ensuring compliance with company policy and all federal, state, and local regulating bodies and conditions of participation and remaining informed and updated on home health regulations and standardsEnsuring documentation is accurate and complete within each client's chartDeveloping, implementing, reviewing, and revising policies and procedures to guide care deliveryCoordinating all client services by reviewing plans of treatments, clinical and progress notes, and frequency of careCommunicating with physician's offices, discharge planners, hospitals, skilled nursing facilities, and families regarding the client's care and statusAuditing charts on a regular basis to assure proper maintenance of clinical recordsPerforming other such tasks and assignments as directed by management/leadership of organizationEducation/skills/abilities/availability include:Bachelor of Science degree in nursing (BSN)Registered nurse licensure in the State of GATwo (2) years of work experience in Home Health CareValid CPR, First Aid, AEDValid driver's license, working vehicle, and auto insuranceAbility to make recommendations to address and resolve issuesAbility to effectively deal with diverse groups and individuals and to maintain working relationshipsAbility to organize workload and set prioritiesAbility to operate office equipment, including, but not limited to computer, printers, copiers, and fax machines. Knowledge of modern office practicesKnowledge and skill in records administration and maintenanceWorking environment:Works primarily out of the local office.
