The Role
We are seeking a full-time Functional Medicine Provider (NP, PA, DO, or MD) to deliver high quality care inside systems that are already built and already working. You will not be figuring out the model. You will be practicing medicine.
The right person brings steady leadership, communicates with clarity and kindness, partners closely with our functional nutritionist, and helps clients take ownership of their own health.
Full-time, in-person, with occasional telemedicine visits.
What You'll Do
- Lead 6 to 8 comprehensive client visits per day , delivering science based care that treats causes instead of symptoms
- Build and adjust personalized treatment plans from labs, symptoms, and client progress
- Partner with our functional nutritionist and provider team in Weekly Case Review Sessions
- Support clients with clear communication, healthy boundaries, and grounded emotional presence
- Lead or co lead Workshops, AMA sessions, and community events (two evenings per month)
- Complete clinical documentation within 24 hours
- Contribute to a culture built on ownership, humility, clarity, honesty, and growth
What Success Looks Like
- High client satisfaction and consistent progress across treatment phases
- 85% to 90% client adherence to treatment plans
- A strong partnership with the Functional Nutritionist and the clinical team
- 12 to 18 client testimonials generated in your first year
- Full integration into Upstream's systems and cultural values
What You Bring
Required
- NP, PA, MD, or DO with 3+ years of clinical experience
- Functional Medicine certification, or actively pursuing one
- Strong functional lab interpretation skills
- Comfort prescribing supplements, hormones, and medications
- Clear, direct communication and healthy boundaries
- The ability to navigate crucial conversations gracefully
- In person availability, with some telemedicine as needed
- The ability to lead client education
Preferred
- Additional functional medicine training
- Experience in functional or integrative medicine
- Expertise in hormones, detox, gut health, toxicity, or autoimmune care
- A coaching or motivational interviewing background
- Strong nutrition knowledge
- A passion for leading group education, in person or online (public speaking experience helps but is not required)
Tools & Systems
Cerbo, Charm, Elation, or a similar EHR. Secure messaging platforms. Functional lab portals including Genova, GPL, DSL, and Vibrant America. Presentation and facilitation tools. Google Workspace and Loom.
Schedule
- Full time: 36 to 40 hours per week
- Clinic hours: Monday to Thursday, 8:30am to 5:30pm MT
- Optional flex hours on Fridays
- No remote or contract options
- Open to discussing a relocation package
- $100,000 to $140,000 annual salary, depending on functional medicine training and experience
