We are seeking an experienced and compassionate Hospice PRN Case Manager who is dedicated to providing exceptional end-of-life care with dignity, empathy, and clinical excellence, and who is willing to travel in DFW Area.
nThis role is ideal for a nurse who is passionate about providing high-quality, patient-centered end-of-life care while enjoying flexibility in scheduling. You will work closely with patients, families, and an interdisciplinary team to ensure comfort, dignity, and support throughout the hospice journey.

nDallas Metro Area
nKey Responsibilities:
nConduct comprehensive patient assessments and develop individualized care plans
nProvide skilled nursing care and symptom management
nCoordinate care with physicians, social workers, chaplains, and other team members
nEducate patients and families on disease progression, medications, and care techniques
nMonitor patient condition and adjust care plans as needed
nMaintain accurate and timely clinical documentation
nParticipate in on-call rotation as needed
nEnsure compliance with hospice regulations and standards
nQualifications:
nCurrent and unrestricted RN license in the state of Texas
nMinimum 1–2 years of hospice, home health, or related experience preferred
nStrong clinical assessment and critical thinking skills
nExcellent communication and interpersonal abilities
nAbility to work independently and manage time effectively
nReliable transportation and valid driver’s license
nCurrent CPR certification (required)
