Full-Time RN Case Manager

Role Overview

The RN Case Manager provides compassionate end-of-life hospice care in patients' homes or community settings. Responsibilities include assessing, planning, implementing, and evaluating patient care, supervising hospice aide staff, and coordinating services to support patients and families during sensitive times. The role involves managing a patient caseload, participating in interdisciplinary team meetings, and ensuring documentation and communication with healthcare teams. The RN develops individualized care plans addressing patient needs, incorporating the 4Ms (What Matters, Medications, Mentation, Mobility) and providing Age-Friendly Care. The position requires strong clinical judgment, excellent communication skills, and empathy to support patients and families effectively.

Qualifications & Skills

• Current unrestricted RN licensure in the state of practice
• Valid driver's license and auto insurance
• Minimum of two years RN experience, preferably in hospice or palliative care

Key Responsibilities

• Manage patient/family caseload, including assessments and care planning
• Supervise and instruct hospice aide staff
• Coordinate medication and supply delivery outside regular hours
• Participate in interdisciplinary team meetings during on-call periods
• Admit eligible patients for hospice care under physician orders
• Report patient status and coordinate with healthcare teams
• Ensure documentation and care notes are complete and accurate
• Deliver patient-centered care aligned with core values of compassion and integrity
