- Conduct in-home assessments and develop individual care plans.
- Skilled nursing & ongoing oversight.
- Assist families with services, rates and long-term care insurance.
- Documentation & Supervisory visits for caregivers and nurses.

Qualifications:
- FL RN license
- Driver’s License
- Current CPR Card
- Experience in Home Health/Private Duty preferred
- Reliable transportation/Auto Insurance
- Level 2 background check required
- Certificate from Alzheimer’s 2 hr course (Dept of elder affairs)
- 1 hr training video Alzheimer’s (Dept of elder affairs)
- Physical/TB done within last 6 months
