About ElderTree Care Management
ElderTree Care Management partners with older adults and their families to navigate aging with clarity, dignity, and confidence. We support clients through complex medical, cognitive, and life transitions by coordinating care, advocating across systems, and serving as a trusted guide during some of life’s most difficult moments.
What Is Care Management?
Care management is a holistic, relationship-driven service that helps older adults and their families manage health, safety, and quality-of-life needs as they age.
At ElderTree, Care Managers:
- Assess medical, cognitive, functional, and psychosocial needs

- Coordinate with physicians, therapists, facilities, and home care providers
- Advocate for clients and families across healthcare and community systems
- Provide ongoing guidance, monitoring, and problem-solving as needs evolve
- Work across the metro DC area to provide clinical advocacy and psychosocial support for our clients
If you enjoy clinical thinking without bedside burnout, value autonomy, and want to make a meaningful impact over time, this role may be a strong fit!

Our hours of operation are Monday - Friday, 9 am - 6 pm EST, with the expectation of being on a rotating on call schedule for periodic weekends. Individuals must drive a car, have a good driving record, and be able to see clients across the DMV (mileage & tolls reimbursed)
