We're seeking a skilled and empathetic Full Time Hospice Case Manager to join our dynamic team in the Flint and surrounding areas. Key Responsibilities
- Conduct initial and ongoing assessments of patients' needs to develop comprehensive care plans
- Provide emotional support and education to patients and families about end-of-life care and self-care techniques
- Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress
- Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care
- Maintain accurate clinical documentation and participate in performance improvement initiatives
Requirements
- Current Michigan Registered Nurse (RN) License in good standing
- Minimum of 1 year experience in hospice or palliative care is a plus
- Ability to work collaboratively with an interdisciplinary care team
- Empathetic and compassionate approach with patients and families
- Current driver's license
- Eligible to work in the United States
- Able to travel across Flint, Lapeer, and surrounding areas for home visits
- Willingness to participate in weekend on-call duties on a rotating basis
- Reliable personal vehicle for home visits across the surrounding areas
