Fresno VACCHCS & UCSF Fresno - Physician - Palliative Care - Internal Medicine

Position Description

VACCHCS Medicine Service/UCSF Fresno Department of Medicine is recruiting a full-time board certified or board eligible Internal Medicine physician trained in palliative care. The physician must have completed an internal medicine residency, hold an MD, DO or equivalent, be eligible for a US medical license, and be eligible for appointment at the VA. The Palliative Care position is responsible for ensuring high quality practices in the treatment of patients at the end of life through coordination and integration of patient care and various nursing, medical and complementary interventions.

The role includes coordination of multidisciplinary care across inpatient, outpatient and community settings and serves as a consultant and resource for educational programs and metrics to monitor program effectiveness. The position will also support patients and families in active participation in care management and lifestyle adjustments related to health.

VACCHCS is a complexity level II health care facility with an active clinical research program.

Responsibilities

• Coordinate multidisciplinary efforts in caring for patients at the end of life across the continuum of care.
• Serve as consultant and resource person for development and implementation of educational programs.
• Develop and implement measures to monitor program effectiveness.
• Assist patients and significant others to assume active participation in management of their care.
• Support the UCSF Fresno residency and fellowship programs.

Qualifications

• Board certified or board eligible Internal Medicine physician trained in palliative care.
• Completed internal medicine residency.
• MD, DO or equivalent and eligible for a U.S. medical license.
• Eligible for appointment at the VA.

Salary estimate: $240,000-$315,000.
