WHO WE ARE
Open Door Community Health Centers was established in 1971. For nearly 50 years, we have met our populations healthcare needs, and are the medical home to over 60% of the community. We are a private, non- profit FQHC with stable and innovative management, and we strive to offer comprehensive care and outstanding service. We take an integrated approach to health care, focusing on prevention, self-management, and health maintenance by engaging each patient as a key member of the care team.

We are located among the giant redwoods on California's North Coast between the Trinity Alps and the Pacific Ocean. Requirements
EDUCATION and/or EXPERIENCE
Diploma from an accredited medical school leading to licensing as a Physician Assistant or Nurse Practitioner.
CERTIFICATES, LICENSES, REGISTRATIONS
- Current California Physician Assistants or Nurse Practitioners license and Board Certification.
- Current DEA license.
- Current CPR license.
SUPERVISORY RESPONSIBILITIES

PHYSICAL REQUIREMENTS
- Speaking and hearing sufficient to communicate effectively by phone or in person, at normal volumes.
- Vision adequate to read correspondence, computer screen, forms, etc.
- Good manual dexterity.
- Ability to sit or stand for extended periods of time.
- Must possess a valid driver's license and reliable means of transportation.
