To support a growing population health team, the full-time Florida RN Case Manager will coordinate comprehensive patient care, advocate for patients, and engage with a multidisciplinary team while working remotely. Key responsibilities Conduct initial intake calls and ongoing patient outreach to update care plans and monitor progress Provide care coordination for a panel of complex patients and develop plans of care in collaboration with the care team Support patients during care transitions and actively participate in interdisciplinary team meetings Required qualifications Current RN License in good standing in Florida Bachelor's degree in nursing or an associate's in nursing with another bachelor's degree Minimum of 4 years of clinical experience, including at least 2 years in case management Certified Case Manager (CCM) certification or commitment to obtain when eligible Geriatric care experience is highly desired
